A collaborative approach to outpatient care can be efficient and cost-effective in helping patients make positive change in managing their chronic illness while also easing demands on the health professional’s time.  

Assessment of psycho-social functioning allows customized interventions for patients and caregivers along a wide spectrum. 

 

  • Short term counseling can shed light on financial, relational, emotional and other barriers to wellness. 
  • Care management can offer support and practical strategies to help overwhelmed patients and caregivers maximize their resources and those in their communities. 
  • Patients who are well-informed about their illness and are apprised of options are more likely to be treatment compliant and to do better.
